There are
forms to be used by departments requesting our services and by RE to obtain approvals
to complete leasing transactions. Copies of the forms can be obtained on this
web page by clicking on "Forms."
For
new leases or lease renewals, complete the "Initiation of Services for Lease of Premises"
form described below. Further information can be obtained by calling Helen Levay at 510.643.2066. " Initiation of Services for Lease of Premises "
Form The
form must be approved by the Campus Department's Chair, Dean or Director, and
reviewed by Space
Management & Capital Programs. The
form will give RE the information it requires to do a preliminary site
search, estimate the real estate costs to the department and estimate the
recharge cost for the project. It will also guarantee that all appropriate
offices are aware of projects being initiated, and determine if there is
appropriate space in any Campus-owned buildings off-campus. The
completed form can be scanned and emailed to Helen Levay
(hlevay@berkeley.edu)
or faxed to Real Estate Services at 510.643.1804. Departments
will be assessed a recharge for canceled projects, based
upon time spent prior to cancellation. "
Authorization for Lease of Premises" Form After RE and
the department have identified an appropriate leased space and RE has
negotiated the terms of the lease transaction, it will complete the "Authorization
for Lease of Premises". This
form will be used to identify the terms and conditions of the lease as
negotiated, and will be reviewed and approved as follows: Leases
to be signed by the campus -
Upon
receipt of all necessary signatures, RE will either execute the lease, or
have it executed by Office
of the President or The Regents, as required. Campus has signing authority
for leases of not more than 10 years excluding options and that are on a standard
form agreement or an agreement that has been reviewed by Office
of the President Real Estate Services Group and Office
of General Counsel. |
